RESUMO Relato de caso não descrito na literatura oftalmológica nacional de múltiplas camadas da membrana de Descemet. Mulher de 59 anos, pseudofácica, com diagnóstico de ceratopatia bolhosa, foi submetida à ceratoplastia penetrante em olho direito, sendo encontrado achado incomum de multiplicidade de camadas da membrana de Descemet, variáveis em forma e espessura, além de corpos ovoides com coloração de metamina de prata de Gocott-Gomori (GMS) negativa. Como a membrana de Descemet tem no seu desenvolvimento um período fetal e outro pós-natal que frequentemente é bem identificável nos cortes histológicos em adultos. No caso apresentado, em que outras camadas estão presentes, há evidência de corpos esféricos que poderiam significar que a potencialidade de produzir outras camadas pode permanecer na fase adulta mais tardia.


ABSTRACT To relate a case not described previously in the national multilateral ophthalmological literature of Descemet's membrane. A 59-year-old pseudophakic woman diagnosed with bullous keratopathy underwent penetrating keratoplasty with an unusual finding of multiple layers of Descemet's membrane, which were variable in shape and thickness. In addition, ovoid bodies with negative Gocott-Gomori (GMS) color were present. Discussion and Conclusion: As Descemet's membrane has a fetal and a postnatal period in its development, it is frequently and well identifiable in histological sections in adults. In the case presented, in which other layers are present, there is evidence of spherical bodies that could mean that the potential to produce other layers may remain into later adulthood.

RESUMO Objetivo: Descrever o perfil clínico, cirúrgico e geográfico de pacientes acompanhados em um hospital universitário e submetidos a transplante de tecido corneano penetrante, com descrição das indicações para o procedimento e do tempo médio na fila de espera. Métodos: Estudo transversal e retrospectivo, incluindo 40 olhos de 40 pacientes submetidos à ceratoplastia penetrante no período de 1° de janeiro de 2018 a 31 de dezembro de 2019, acompanhados em um hospital universitário em Alagoas. Os dados foram coletados por meio das fichas de informações cirúrgicas dos transplantes de córnea da Central de Notificação, Captação e Distribuição de Órgãos e Tecidos de Alagoas, adaptados aos objetivos da pesquisa. Resultados: Dos transplantes de córnea estudados, 52,5% (n=21) foram realizados em pacientes do sexo feminino, 62,5% (n=25) na faixa etária acima de 60 anos, com média de idade de 59,17 anos (±20,4). Todos ocorreram em pacientes que residiam no estado de Alagoas, sendo 60% deles na região intermediária de Maceió. As principais indicações para o transplante de córnea foram ceratopatia bolhosa do pseudofácico (n=11; 27,5%), descemetocele (n=9; 22,5%) e falência tardia do enxerto (n=6; 15%). Dentre os procedimentos, 70% foram realizados no olho esquerdo (n=28) e 65%(n=26) com propósito óptico; houve associação de extração de catarata em 22,5% (n=9), e 5% (n=2) apresentaram complicações peroperatórias. O tempo médio geral em lista de espera foi de 332,3 dias (11 meses), sendo 486 dias (2 anos e 4 meses) para cirurgias eletivas e 12,8 dias para as de urgência. Conclusão: O tempo de espera para cirurgias eletivas foi longo e inadequado. A principal condição indicadora para o transplante de córnea foi a ceratopatia bolhosa. O conhecimento do perfil dos transplantes de córnea pode permitir a identificação de grupos de risco para fins de prevenção e implementação de cuidados, que resultem em prognósticos mais favoráveis, bem como incentivar a implementação de políticas internas e externas para melhoria do sistema captação-doação.


ABSTRACT Objective: To describe the clinical, surgical, and geographic profile of patients submitted to corneal transplantation and followed up at a teaching hospital, and, with a description of the indications for the procedure and mean waiting time. Methods: A cross-sectional and retrospective study, including 40 eyes of 40 patients who underwent penetrating keratoplasty, from January 1, 2018 to December 31, 2019, followed up at a teaching hospital in Alagoas State. Data were collected using the surgical records of corneal transplants, from the Central Reporting, Procurement and Distribution of Organs and Tissues of Alagoas, and adjusted to the research objectives. Results: Of the corneal transplants studied, 52.5% (n=21) were performed in female patients; 62.5% (n=25) in the age group over 60 years; with a mean age of 59.17 years (± 20.4). All transplanted patients lived in the state of Alagoas, 60% of them in the intermediate metropolitan region of Maceió. The main indications for corneal transplantation were pseudophakic bullous keratopathy (27.5%, n=11), descemetocele (22.5%, n=9) and late graft failure (15%, n=6). Seventy percent (n= 28) of procedures were performed on the left eye and 65% (n=26) for optical purposes. There was an association of cataract extraction in 22.5% (n=9), and 5% (n=2) had perioperative complications. The mean waiting list time was 332.3 days (11 months); in that, 486 days (2 years and 4 months) for elective surgeries and 12.8 days for emergency surgeries. Conclusion: The waiting time for elective surgeries was long and inappropriate. The major indication for corneal transplantation was bullous keratopathy. Knowledge of the clinical profile of corneal transplants can enable identifying the risk groups for prevention and implementation of care, resulting in better prognosis, fostering implementation of internal and external policies to improve the procurement-donation system.

RESUMEN La queratoplastia penetrante ha sido el procedimiento empleado, de forma casi universal, para el tratamiento de las patologías corneales que afectan de forma irreversible a la visión. Se reportaron 4 casos clínicos de pacientes con afecciones corneales con compromiso visual severo, dado por visión de cuenta dedos y percepción luminosa. Luego de la evaluación completa en cada caso, se decidió realizar la queratoplastia penetrante con fines ópticos, con el objetivo de recuperar la transparencia de la córnea y la calidad visual. No existieron complicaciones transquirúrgicas y fueron seguidos según protocolo de evaluación postoperatoria de trasplante corneal. Los resultados y evaluación fueron satisfactorios, al año mantenían la transparencia del injerto y se logró mejoría considerable de la agudeza visual (AU).


ABSTRACT Penetrating keratoplasty has been a procedure used, almost universally, to treat corneal diseases affecting vision in an irreversible way. Four clinical cases are reported of patients with corneal affections severely compromising vision, given by short vision and luminous perception. After completely evaluating each case, the authors decided to perform the penetrant keratoplasty with optical aims, to recover cornea transparency and the vision quality. There were not transurgical complications and the patients were followed up according to the protocols of post-surgery evaluation of corneal graft. The results and evaluation were satisfactory; after a year, the transparency of the graft was maintained and visual acuity substantially improved (AU).

ABSTRACT Purpose: To evaluate causes and management of congenital corneal opacities (CCO) diagnosed in a tertiary care eye center and to compare the data with a previous study at the same institution. Methods: Computerized medical records in all patients with congenital corneal opacities diagnosed in the Cornea Service at Wills Eye Hospital (Philadelphia, PA) between January 1, 2007, and December 31, 2015, were retrospectively reviewed. Children aged 12 years and younger at the first visit were included in the study. Patients' demographics, ocular diagnosis, laterality, associated ocular abnormalities, other ocular surgery performed prior or subsequent to the first visit, and their treatment were extracted from the medical records. Results: A total of 77 eyes in 56 patients were examined. The mean age at presentation was 32.8 ± 44.2 months, with the mean follow-up period of 26.7 ± 30.1 months. The most frequent diagnosis was Peters anomaly (53.2%), followed by limbal dermoid (13.0%), aniridia with glaucoma and microphthalmos (6.5%), sclerocornea and congenital glaucoma (5.2%), idiopathic (3.9%), Axenfeld-Rieger anomaly and Hurler syndrome (2.6%), and microcornea (1.3%). Primary keratoplasty was performed in 26 eyes, with the outcome rate in the clear cornea of 76.0% during the follow-up. Conclusion: Peters anomaly is the most common cause of congenital corneal opacities encountered at our institution. Penetrating keratoplasty is the most frequent choice of corneal surgery to treat congenital corneal opacities. Additional interventions during penetrating keratoplasty were moderately positively correlated with graft failure. This study also shows the rates of some etiologies of that changed over the recent decades in our tertiary care Cornea Service. Although Peters anomaly remains the most common presenting reason for congenital corneal opacities, its rate appears to be increasing over the recent decade. Congenital corneal opacities due to birth trauma, which is one of the preventable causes, were observed in a previous study in our clinic; however, no new cases were noted in this study.


RESUMO Objetivo: Avaliar as causas e o controle das opa cidades corneanas congênitas diagnosticadas em um centro oftal mológico de atendimento terciário e comparar os dados com um estudo anterior realizado na mesma instituição. Métodos: Prontuários médicos informatizados de todos os pacientes com opacidade corneana congênita diagnosticada no Serviço de Córnea no Wills Eye Hospital (Filadélfia, PA) entre 1º de ja neiro de 2007 e 31 de dezembro de 2015 foram revisados retrospectivamente. Crianças com 12 anos ou menos na primeira consulta foram incluídas no estudo. A demografia dos pacientes, o diagnóstico ocular, a lateralidade, as anormalidades oculares associadas, outras cirurgias oculares realizadas antes ou após a primeira consulta e o tratamento foram extraídos dos prontuários médicos. Resultados: Um total de 77 olhos de 56 pacientes foi examinado. A idade média de apresentação foi de 32,8 ± 44,2 meses, com um tempo médio de acompanhamento de 26,7 ± 30,1 meses. O diagnóstico mais frequente foi anomalia de Peters (53,2%), seguido por dermóide límbico (13,0%), aniridia com glaucoma e microftalmia (6,5%), esclerocórnea e glaucoma congênito (5,2%), idiopático (3,9%), síndrome de Axenfeld-Rieger e síndrome de Hurler (2,6%) e microcórnea (1,3%). Ceratoplastia primária foi realizada em 26 olhos, com desfecho de córnea clara de 76,0% durante o acompanhamento. Conclusão: A anomalia de Peters é a causa mais comum de opacidade corneana congênita encontrada em nossa instituição. A ceratoplastia penetrante é a escolha mais frequente de cirurgia corneana para o tratamento de opacidades corneanas congênitas. Intervenções adicionais durante a ceratoplastia penetrante foram moderadamente correlacionadas positivamente com a falha do enxerto. Este estudo também mostra as taxas de algumas etiologias do que mudou ao longo faz últimas décadas em nosso serviço de córnea de atendimento terciário. Embora a anomalia de Peters continue a ser a causa mais comum das opacidades congênitas da córnea, sua taxa parece estar aumentando na última década. Opacidades congênitas da córnea devido a trauma no nascimento, que é uma das causas evitáveis, foram observadas em um estudo anterior em nossa clínica; no entanto, nenhum caso novo foi observado neste estudo.

RESUMEN Objetivo: Comparar los resultados visuales de la queratoplastia lamelar anterior profunda versus la queratoplastia penetrante en el tratamiento quirúrgico del queratocono. Métodos: Se realizó un estudio analítico longitudinal prospectivo en 81 pacientes con queratocono, operados de queratoplastia penetrante y queratoplastia lamelar anterior profunda entre los años 2014 y 2018. Se evaluó la edad, el sexo, la agudeza visual corregida, la esfera, el cilindro y el equivalente esférico. Resultados: La media de edad en operados de queratoplastia lamelar anterior profunda fue 27,5 ± 10,0 años, y 33,2 ± 12,9 años en operados de queratoplastia penetrante. En ambos grupos predominó el sexo masculino: 62,5 por ciento en el grupo de queratoplastia lamelar anterior profunda y 63,3 por ciento en el de queratoplastia penetrante. La agudeza visual corregida poscirugía en operados de queratoplastia lamelar anterior profunda fue 0,8 ± 0,19; el cilindro refractivo -3,50 ± 1,50; el equivalente esférico -6,07 ± 3,61, SimK1 de 46,37 ± 2,47 dioptrías y SimK2, 43,05 ± 2,47 dioptrías. En la queratoplastia penetrante, la agudeza visual corregida fue 0,68 ± 0,19; el cilindro refractivo -2,77 ± 0,98 dioptrías; el equivalente esférico -3,68 ± 1,56 dioptrías, SimK1 y SimK2 de 44,58 ± 2,77 dioptrías y 41,40 ± 2,93 dioptrías respectivamente. Conclusiones: Los resultados refractivos son similares en ambas técnicas quirúrgicas(AU)


ABSTRACT Objective: Compare the visual results of deep anterior lamellar keratoplasty versus penetrating keratoplasty in the surgical treatment of keratoconus. Methods: A longitudinal analytical prospective study was carried out in 81 patients with keratoconus, operated of penetrating keratoplasty and deep anterior lamellar keratoplasty between the years 2014 and 2018. The variables analyzed were the age, the sex, the corrected visual sharpness, the sphere, the cylinder and the spherical equivalent. Results: The average of age in operated patients of deep anterior lamellar keratoplasty was 27,5 ± 10,0 years, and 33,2 ± 12,9 years in operated of penetrating keratoplasty. In both groups the masculine sex predominated: 62,5 percent in the group of deep anterior lamellar keratoplasty and 63,3 percent in penetrating keratoplasty. The corrected visual sharpness in operated of queratoplastia lamelar previous deep was 0,8 ± 0,19; the refractive cylinder -3,50 ± 1,50; the spherical equivalent -6,07 ± 3,61, SimK1 of 46,37 ± 2,47 dioptres and SimK2, 43,05 ± 2,47 dioptres. In the penetrating keratoplasty, the corrected visual sharpness was 0,68 ± 0,19; the refractive cylinder -2,77 ± 0,98 dioptres; the spherical equivalent -3,68 ± 1,56 dioptres, SimK1 and SimK2 of 44,58 ± 2,77 dioptres and 41,40 ± 2,93 dioptres respectively. Abstract Background: Penetrating keratoplasty (PK) is a challenging surgical ocular procedure indicated for some pediatric patients with vision-threatening corneal disease damage. Pediatric PK is reserved for cases with advanced pathology or dysfunction as rejection and failure rates greatly supersede the rates of adult PK. The objective was to identify factors associated with graft rejection and failure amongst Mexican children undergoing primary PK for different indications. Methods: A retrospective review of consecutive pediatric PK medical records was conducted between 2001 and 2015 at the Hospital Infantil de México Federico Gómez. Graft survival was calculated using the Kaplan–Meier survival method. Results: During the 15 years of study, 67 eyes of 57 patients underwent PK. The mean age at the time of surgery was 11 years. The mean follow-up duration was 44 months. Keratoconus was the most common indication for PK (61%) followed by herpetic keratitis (15%) and other corneal dystrophies (10%). Mean graft survival time was 45.6 months (95% confidence interval 31.8-58.4 months, standard deviation = 0.069), with a survival rate of 70% at 1 year. Univariate Cox proportional hazard showed that being < 9 years of age at the time of the surgery (p = 0.023) and corneal dystrophies (p = 0.04) were prognostic factors for corneal rejection. Five eyes of five patients developed graft failure. Conclusions: Pediatric PK patients commonly experience graft rejection, which need to be promptly diagnosed and treated to minimize permanent damage. The age at the time of surgery and congenital corneal opacities is factors associated with a higher risk of rejection and failure.


Resumen Introducción: La queratoplastia penetrante (QP) es un procedimiento quirúrgico indicado en algunos pacientes pediátricos con daño corneal con riesgo de fracaso que amenaza la visión. El objetivo del presente trabajo fue identificar los factores asociados a rechazo y fallo del injerto corneal en niños sometidos a QP. Métodos: Serie de casos de pacientes sometidos a QP en el Hospital Infantil de México Federico Gómez entre 2001 y 2015. La sobrevida del injerto fue calculada por el método de estimación de la función de supervivencia Kaplan–Meier. Resultados: Durante 15 años de estudio, 67 ojos de 57 pacientes fueron sometidos a QP. El promedio de edad de los pacientes al tiempo de la cirugía fue de 11 años. El seguimiento promedio fue de 44 meses. El queratocono fue la indicación más común de QP (61%), seguido de queratitis herpética (15%) y otras distrofias corneales (10%). El tiempo promedio de sobrevida fue 45.6% (intervalo de confianza [IC] 95%: 31.8-58.4 meses, desviación estándar [DS] = 0.069), con una tasa de sobrevida del 70% a 1 año. El riesgo proporcional de Cox univariado mostró que una edad < 9 años al momento de la cirugía (p = 0.023) y distrofias corneales (p = 0.04) son factores pronóstico para el rechazo corneal. Cinco ojos de cinco pacientes desarrollaron falla del injerto. Conclusiones: El rechazo del injerto en QP en pacientes pediátricos es común y puede ser resuelto satisfactoriamente en casi todos los casos con diagnóstico y tratamiento. La edad al momento de la cirugía y las opacidades corneales congénitas son factores asociados con el rechazo y fallo.

Objetivo identificar as principais complicações intraoperatórias dos pacientes que realizaram ceratoplastias e sua relação com fatores clínicos e cirúrgicos. Método estudo transversal, observacional. Realizou-se um censo dos pacientes submetidos a ceratoplastias que totalizou 258 procedimentos. Resultados foram registradas 22 complicações intraoperatórias, todas em ceratoplastias penetrantes. Do total, 59,09% foram realizadas em pacientes do sexo masculino com idade média de 58,5 anos. A principal complicação intraoperatória notificada foi a perda vítrea (36,36%). Encontrou-se relação estatisticamente significativa entre a variável "complicação intraoperatória" e as variáveis "cirurgia prévia", "ceratoplastia combinada com extração de catarata" e "botão corneano do receptor maior que 8,0 mm". Conclusão identificar as principais complicações intraoperatórias da ceratoplastia possibilita à enfermagem compreender quais fatores podem interferir nesses procedimentos, apontar possíveis fatores preditores das complicações e buscar medidas de controles para que tais complicações não ocorram.


Objective to identify the main intraoperative complications of patients who underwent keratoplasty and relationship between these complications and clinical and surgical factors. Method cross-sectional observational study. A census of the patients submitted to keratoplasty was carried out, which totaled 258 procedures. Results twenty-two intraoperative complications were recorded, all in penetrating keratoplasty surgeries, of which 59.09% were performed in male patients with a mean age of 58.5 years. The main intraoperative complication was vitreous loss (36.36%). A statistically significant relationship was found between the variable "intraoperative complication" and the variables "previous surgery", "combined keratoplasty and cataract extraction" and "corneal host button greater than 8.0 mm". Conclusion identifying the main intraoperative complications of keratoplasty enables nurses to understand which factors may interfere with these procedures, point out possible predictors of complications, and seek control measures so that such complications do not occur.


Objetivo identificar las principales complicaciones intraoperatorias de los pacientes que realizaron queratoplastias y su relación con factores clínicos y quirúrgicos. Método estudio transversal, observacional. Se realizó un censo de los pacientes sometidos a las queratoplastias que tuvo un total de 258 procedimientos. Resultados fueron registradas 22 complicaciones intraoperatorias, todas en queratoplastias penetrantes. Del total, 59,09% fueron realizadas en pacientes del sexo masculino con edad media de 58,5 años. La principal complicación intraoperatoria notificada fue la pérdida vítrea (36,36%). Fue encontrada relación estadísticamente significativa entre la variable "complicación intraoperatoria" y las variables "cirugía previa", "queratoplastia combinada con extracción de catarata" y "botón corneal del receptor mayor que 8,0 mm". Conclusión identificar las principales complicaciones intraoperatorias de la queratoplastia posibilita a la enfermería comprender cuáles factores pueden interferir en estos procedimientos, apuntar posibles factores predictores de las complicaciones y buscar medidas de controles para que tales complicaciones no ocurran.

Introducción: el trasplante de córnea es el injerto mas frecuente en el campo de la medicina; las técnicas para realizarlo han evolucionado permitiendo hacer procedimientos menos invasivos, con menor riesgo de rechazo del injerto y con una recuperación más rápida. La escasez de donantes para realizar trasplantes de córnea, genera una limitación terapéutica muy importante en el campo de la patología corneal. Objetivo: Reportar las Indicaciones y Técnicas empleadas en los Trasplantes de Córnea, en una institución privada con atención terciaria en Oftalmología en Bogotá, Colombia. Diseño del Estudio: Estudio transversal de fuentes secundarias. Método: Se hizo un estudio transversal, con las historias clínicas electrónicas de los pacientes operados con algún tipo de trasplante de córnea, en la Clínica Barraquer de América en el periodo comprendido desde Enero del 2010 a Diciembre del 2018 Resultados: Las cuatro primeras Indicaciones para Trasplantes de córnea fueron: 1) Ectasias Corneales 38.27% 2) Descompensación corneal 20.88% 3) Reposición de Injerto 17.72% 4) Leucomas Cicatriciales 16.22%. Las técnicas empleadas fueron Injerto Penetrante el 54.11% - Injertos Laminares (43.52%) con un 17.25% de Injertos Endoteliales. Conclusión: Existen diferencias en la frecuencia de las Indicaciones para trasplante de córnea según la región del país y también respecto a las publicaciones internacionales. El Queratocono fue la indicación mas frecuente. La Queratoplastia Penetrante la técnica más frecuente como procedimiento primario, pero también como técnica alternativa debido a la falta de oportunidad para realizar el trasplante.


Background: corneal transplant is the most frequent graft in the fi eld of medicine; the techniques to carry it out have evolved allowing to make less invasive procedures, with less risk of rejection of the graft and with a faster recovery. The shortage of donors to perform corneal transplants generates a very important therapeutic limitation in the field of corneal pathology.. Objective: To report the Indications and surgical techniques in Corneal Transplants in a tertiary referral center and private Ophthalmological clinic in Bogotá-ColombiaStudy Design: Cross sectional study using secondary data. Method: A cross sectional study was performed with the electronic clinical records of the patients that had a corneal graft of any kind during the period between January 2010 and December 2018 at the Barraquer Clinic in América. Results: The top four indications for corneal transplantation were: 1) Corneal Ectasias 38.27% - 2) Bullous Keratopathy 20.88% ­ 3) Re-Graft 17.72% and 4) Leucomas 16.22%. Penetrating Keratoplasty was the most common indication (54.11%) followed by Lamellar Techniques 43.52% of which Endothelial keratoplasty was performed in 17.25%. Conclusion: Corneal transplant indications and its frequency, are different between country regions in Colombia and towards international reviews. For us, Keratoconus was the most frequent indication. and Penetrating Keratoplasty, the most frequent technique as a primary procedure, but also as an alternative technique due to the lack of opportunity to perform the transplant.

ABSTRACT We report a case of central corneal perforation treated with an autologous lamellar scleral graft and histologic findings obtained after a subsequent penetrating keratoplasty. A corneal perforation within a large Pseudomonas ulcer in a 55-year-old male rigid gas permeable contact lens wearer was sealed by a lamellar scleral graft from the same eye, followed by an uneventful penetrating keratoplasty 6 months later. Histology of the excised button revealed that the well-apposed graft, which maintained the irregular arrangement of the scleral collagen fibers, was embedded in the corneal stroma over the deep blood vessels and a rupture in Descemet's membrane. The clinical and histologic findings showed that autologous lamellar scleral grafts can be successfully used for the emergency treatment of corneal perforation when a corneal transplant is not available. The distinctive scleral structure revealed by histology and the inadequate graft transparency indicate that visual rehabilitation of eyes with a central corneal perforation can be achieved only by a subsequent optic penetrating keratoplasty.


RESUMO Relatamos um caso de perfuração corneana central tratada com enxerto autólogo lamelar de esclera e os achados histológicos obtidos após ceratoplastia penetrante (CP) subsequente. Uma perfuração da córnea devido a uma grande úlcera por Pseudomonas em um usuário de lentes de contato rígidas gás permeável de 55 anos de idade foi selada por um enxerto escleral lamelar do mesmo olho, seguida de ceratoplastia penetrante, sem intercorrências, seis meses depois. A histologia do botão excisado revelou que um enxerto bem posicionado, que manteve o arranjo irregular das fibras de colágeno escleral, foi incorporado no estroma corneano sobre os vasos sanguíneos profundos e uma ruptura na membrana de Descemet. Os achados clínicos e histológicos demonstraram que o enxerto autônomo de esclerose lamelar pode ser usado com sucesso como tratamento de emergência da perfuração da córnea, quando o transplante de córnea não é possível. A estrutura escleral característica revelada pela histologia e a transparência inadequada do enxerto indicam que a reabilitação visual dos olhos com uma perfuração corneana central só pode ser alcançada através de uma ceratoplastia penetrante óptica subsequente.

ABSTRACT Purpose: To determine the outcomes of penetrating keratoplasty (PK) for treatment of corneal scarring caused by Herpes simplex virus (HSV) keratitis, and whether the corneal scar type affects treatment outcome. Methods: A retrospective analysis of patients who underwent PK for HSV-related corneal scarring between January 2008 and July 2011 was performed. The patients were categorized into two groups. Group 1 consisted of patients with a quiescent herpetic corneal scar and group 2 consisted of patients who developed a corneal descemetocele or perforation secondary to persistent epithelial defects with no active stromal inflammation. The mean follow-up was 21.30 ± 14.59 months. The main parameters evaluated were recurrence of herpetic keratitis, graft rejection, graft failure, visual acuity, and graft survival rate. Results: There were 42 patients in group 1 and 13 in group 2. Preoperative BCVA varied from hand movements to 0.7 logMAR. Postoperatively, 34 patients (61.8%) achieved visual acuity of 0.6 logMAR or more. Recurrence of HSV keratitis was noted in 12 (28.57%) eyes in group 1 and 4 (30.76%) eyes in group 2 (p=0.40). Graft rejection occurred in 4 eyes (9.52%) in group 1 and in 3 (23.07%) eyes in group 2 (p=0.58). The 1-, 2-, and 3-year graft survival rates were 91.9%, 76.0%, and 65.1% in group 1, and 89.5%, 76.0%, and 63.6% in group 2 (p=0.91), respectively. Conclusions: Although there were different recurrence and graft rejection rates for two groups, the graft survival rates at 3 years were similar. According to our results, without inflammation, corneal herpetic scarring with a descemetocele or perforation achieved similar graft survival rates with quiescent herpetic corneal scars.


RESUMO Objetivo: Determinar os resultados da ceratoplastia penetrante (PK) para o tratamento da cicatriz da córnea consequente à ceratite por Herpes simplex vírus (HSV), e se o tipo de cicatriz na córnea afeta o resultado cirúrgico. Métodos: Foi realizada análise retrospectiva dos pacientes, submetidos à PK para a cicatriz da córnea relacionados com o HSV entre janeiro de 2008 e julho de 2011. Os pacientes foram divididos em dois grupos. Grupo 1 consistiu de pacientes que tiveram cicatriz corneana herpética quiescente e grupo 2 consistiu de pacientes que desenvolveram descemetocele ou perfuração córnea secundária a defeitos epiteliais persistentes sem inflamação estromal ativa. O seguimento médio foi de 21,30 ± 14,59 meses. Os principais parâmetros avaliados foram recorrência de ceratite herpética, rejeição de enxerto, falência do enxerto, acuidade visual e taxa de sobrevida do enxerto. Resultados: Foram avaliados 42 pacientes do grupo 1 e 13 doentes do grupo 2. Acuidade visual pré-operatória variou de movimentos das mãos (HM) para 0,7 logMAR. No pós-operatório, 34 pacientes (61,8%) atingiram acuidade visual de 0,6 logMAR ou melhor. Doze olhos (28,57%) tiveram recorrência de HSV ceratite no grupo 1, e quatro olhos (30,76%) tiveram recorrência no grupo 2 (p=0,40). A rejeição do enxerto ocorreu em 4 olhos (9,52%) no grupo 1, e em 3 olhos do grupo 2 (23,07%; p=0,58), taxa de sobrevivência do enxerto foi de 91,9% a 1 ano, 76,0% aos 2 anos e 65,1% aos 3 anos no grupo 1, e 89,5% a 1 ano, 76,0% aos 2 anos e 63,6% aos 3 anos no grupo 2 (p=0,91). Conclusões: Embora diferentes taxas de recorrência e de rejeição do enxerto foram encontradas nos dois grupos, a taxa de sobrevida do enxerto em 3 anos foi semelhantes nos dois grupos. De acordo com nossos resultados, em casos sem inflamação, a cicatriz herpética da córnea com descemetocele ou perfuração demonstra as taxas de sobrevivência do enxerto semelhantes às da cicatriz corneana herpética quiescente.

ABSTRACT Purpose: The optical quality of the interface after deep anterior lamellar keratoplasty (DALK) using the big-bubble technique has been shown to be excellent, leading to results comparable to penetrating keratoplasty. However, there is little in the literature with respect to the controversy surrounding the preparation of the donor cornea. The purpose of this study was to evaluate visual acuity (VA) in patients with keratoconus who underwent DALK without removal of the donor graft endothelium. Methods: The records of 90 patients who underwent DALK without the removal of the Descemet membrane (DM) and endothelium were retrospectively reviewed. Data collected included uncorrected VA (UCVA) and spectacle-corrected VA (SCVA) at 7, 30, 180 days, and 1 year postoperatively. Contact lens-corrected visual acuity (CLVA) was evaluated after 1 year of the procedure. Results: UCVA was significantly better than preoperative values at 7 days (p<0.001), 30 days (p<0.001), 180 days (p<0.001), and 1 year (p<0.001) after surgery. The 1-year postoperative mean SCVA and CLVA also improved when compared with preoperative SCVA (p<0.001 for both). Conclusions: DALK utilizing donor corneas with attached Descemet membrane and endothelium results in satisfactory VA in patients with keratoconus.


RESUMO Objetivos: A qualidade óptica da interface após ceratoplastia lamelar anterior profunda (DALK) utilizando a técnica de "Big Bubble" mostrou-se ser excelente, levando a resultados comparáveis aos da ceratoplastia penetrante. No entanto, há poucos dados na literatura com respeito à controvérsia em torno da preparação da córnea doadora. O objetivo deste estudo foi avaliar a acuidade visual (VA) em pacientes com ceratocone submetidos DALK sem a remoção da membrana de descemet e endotélio do tecido doador. Métodos: Os prontuários de 90 pacientes que foram submetidos a DALK sem a remoção da membrana Descemet (DM) e do endotélio foram avaliados retrospectivamente. Os dados coletados incluíram VA sem correção (UCVA) e VA corrigida por óculos (SCVA) aos 7, 30, 180 dias, e 1 ano de pós-operatório. A acuidade visual corrigida por lente de contato (CLVA) foi avaliada após 1 ano do procedimento. Resultados: UCVA no pós-operatório melhorou após 7 dias (p<0,001); 30 dias (p<0,001); 180 dias (p<0,001); e após 1 ano (p<0,001). Ocorreu melhora da SCVA pré-operatória quando comparada com a SCVA e CLVA após 1 ano (p<0,001 para ambos). Conclusão: Transplante lamelar anterior utilizando córneas doadas com membrana de Descemet e endotélio demonstrou resultados visuais satisfatórios em pacientes com ceratocone.

ABSTRACT

Objetivo: describir la asociación entre aspectos clinicoepidemiológicos y los resultados microbiológicos en pacientes con queratitis infecciosa ingresados en el Servicio de Córnea del Instituto Cubano de Oftalmología Ramón Pando Ferrer en el período 2010-2014. Métodos: se realizó un estudio observacional descriptivo, de corte transversal, de 39 ojos. Se estudiaron las variables edad, sexo, tiempo de evolución, tratamiento previo con antibióticos, factores predisponentes y concordancia del tratamiento empírico. Se utilizaron medidas de resumen para variables cualitativas y cuantitativas, prueba no paramétrica de probabilidad exacta de Fisher, prueba de Chi cuadrado de Pearson y Prueba U de Mann Whitney. Resultados: en los aislamientos de bacterias, el 42,3 por ciento de los pacientes eran menores de 30 años; 53,8 por ciento pertenecían al sexo femenino y el tiempo de evolución promedio fue de 7 días. Los factores predisponentes fueron: enfermedad ocular previa (23,1 %), uso de lentes de contacto (30,8 por ciento) y enfermedades sistémicas (11,5 por ciento). Los mayores de 69 años (46,5 por ciento), el sexo masculino (92,3 por ciento); el tiempo de evolución promedio de 14,5 días y el antecedente de traumatismo ocular no quirúrgico (15,4 por ciento) se observaron en aislamientos micóticos. La perforación corneal se produjo en el 7,7 por ciento de los crecimientos bacterianos; en crecimientos micóticos el 15,4 por ciento desarrolló descemetocele y el 23,1 por ciento perforación corneal. Se realizó queratoplastia penetrante en el 30,8 por ciento(AU) Conclusiones: los aislamientos bacterianos son más frecuentes en pacientes jóvenes, del sexo femenino y con antecedentes de enfermedad ocular previa y uso de lentes de contacto, mientras que la etiología micótica se produce en pacientes del sexo masculino, mayores de 60 años y con antecedentes de trauma corneal no quirúrgico


Objective: to describe the association of clinical and epidemiological aspects with the microbiological results of patients with infectious keratitis, who were admitted to the corneal service of "Ramon Pando Ferrer" Cuban Institute of Ophthalmology in the period of 2010 to 2014. Methods: cross-sectional, descriptive and observational study of 39 eyes. The analyzed variables were age, sex, time of progression, previous antibiotic treatment, predisposing factors and agreement of the empirical treatment. Summary measures for qualitative and quantitative variables; non-parametric Fisher´s exact probability test, Pearson´s Chi square test, and Mann Whitney's U test. Results: in the bacterial isolates, 42.3 percent of patients were younger than 30 years; 53.8 percent were females and average time of progression of disease was 7 days. The predisposing factors were previous eye disease (23.1 percent), use of contact lenses (30.8 percent) and systemic diseases (11.5 percent). Regarding mycotic isolates, patients aged over 69 years (46.5 percent), males (92.3 percent), average time of progression of 14.5 percent and history of non surgical ocular trauma (15.4 percent) predominated. The corneal perforation occurred in 7.7 percent of bacterial infection whereas 15.4 percent developed descemetocele and 23.1 percent corneal perforation in the mycotic infection group. Penetrating keratoplasty was performed in 30.8 percent of patients. Conclusions: bacterial isolates were more frequent in young female patients with a history of previous ocular diseases and contact lenses whereas mycotic etiology was more commonly observed in males over 60 years and history of non surgical corneal trauma(AU)

ABSTRACT Objective: For nearly a century, penetrating keratoplasty has been the surgical technique of choice in the management of corneal changes. However, in recent years, several lamellar keratoplasty techniques have been developed, modified or improved, especially techniques for replacing the posterior portion, for the correction of bullous keratopathy. The aim of this study was to evaluate the effectiveness and safety of endothelial keratoplasty versus penetrating keratoplasty for pseudophakic and aphakic bullous keratopathy. Methods: A systematic review of the literature was carried out, and the main electronic databases were searched. The date of the most recent search was from the inception of the electronic databases to December 11, 2015. Two authors independently selected relevant clinical trials, assessed their methodological quality and extracted data. Results: The electronic search yielded a total of 893 published papers from the electronic databases. Forty-four full-text articles were retrieved for further consideration. Of these 44 full-text articles, 33 were excluded because they were all case series studies; therefore, ten studies (with one further publication) met the inclusion criteria: one randomized clinical trial with two publications; three controlled studies; and six cohort studies. The clinical and methodological diversity found in the included studies meant that it was not possible to combine studies in a metaanalysis. Conclusions: There is no robust evidence that endothelial keratoplasty is more effective and safe than penetrating keratoplasty for improving visual acuity and decreasing corneal rejection for pseudophakic and aphakic bullous keratopathy. There is a need for further randomized controlled trials.


RESUMO Objetivo: Durante quase um século, a ceratoplastia penetrante tem sido a técnica cirúrgica de escolha no tratamento de doenças corneais. No entanto, nos últimos anos, têm sido desenvolvidas várias técnicas de transplante lamelar, especialmente modificadas ou aperfeiçoadas para substituir a porção posterior para a correção da ceratopatia bolhosa. O objetivo deste estudo foi avaliar a eficácia e segurança da ceratoplastia endotelial quando comparada a ceratoplastia penetrante para a ceratopatia bolhosa afácica ou pseudofácica. Métodos: Uma revisão sistemática da literatura foi realizada, e as principais bases de dados eletrônicas foram pesquisadas. A data das bases de dados da última pesquisa foi 11 de dezembro de 2015. Dois autores selecionaram independentemente os estudos relevantes, avaliaram sua qualidade metodológica e extraíram os dados. Resultados: A busca eletrônica resultou em um total de 893 artigos publicados a partir das bases de dados eletrônicas. Quarenta e quatro artigos de texto completos foram recuperados para uma análise mais aprofundada. Destes 44 artigos de texto completos, 33 foram excluídos por serem estudos de séries de casos, portanto, dez estudos (com uma publicação adicional) preencheram os critérios de inclusão: um ensaio clínico randomizado, três estudos controlados e seis estudos de corte. A diversidade clínica e metodológica encontrada nos estudos incluídos tornou impossível combinar os resultados em uma metaanálise. Conclusões: Não há evidencias robustas de que a ceratoplastia endotelial é mais eficaz e segura do que o transplante penetrante de córnea para melhorar a acuidade visual e diminuir a rejeição da córnea na ceratopatia bolhosa afácica ou pseudofácica. Há necessidade de mais estudos controlados, randomizados.

ABSTRACT

Objetivo: describir las características de queratoplastia pediátrica. Método: se realizó un estudio descriptivo de corte transversal en el Instituto Cubano de Oftalmología Ramón Pando Ferrer. La muestra estuvo conformada por los pacientes en edad pediátrica a quienes se les realizó trasplante de córnea desde el año 1998 hasta el 2012. Las variables estudiadas fueron: edad, sexo, causa del trasplante, objetivo del trasplante y técnica quirúrgica. Resultados: el 35,2 por ciento de los pacientes estaban en rango de edad de 11-15 años seguido del rango 6-10 años. Predominaron los varones (62,6 por ciento). El 89,4 por ciento de los trasplantes fueron por opacidades corneales adquiridas, específicamente el leucoma corneal (58,1 por ciento). El 88,3 por ciento de las queratoplastias fueron penetrantes y el 91 por ciento se realizó con un objetivo óptico. Conclusiones: las queratoplastias pediátricas en el período estudiado fueron principalmente para el restablecimiento visual, por opacidades corneales adquiridas entre los 6 y 15 años de edad. La técnica quirúrgica más empleada fue la queratoplastia perforante(AU)


Objective: to describe the characteristics of pediatric keratoplasty. Method: a descriptive cross-sectional study was conducted in Ramón Pando Ferrer Cuban Institute of Ophthalmology. The sample consisted of pediatric patients who underwent corneal transplantation from 1998 to 2012. The study variables were age, sex, cause and objective of transplantation and surgical technique. Results: in this group, 35,2 percent of patients aged 11 to 15 years-old followed by the 6 to 10 y range. Males predominated (62,6 percent). The cause of 89,4 percent of the transplants was acquired corneal opacities, particularly the corneal leukoma (58,1 percent). Regarding surgical techniques, 88,3 percent were penetrating keratoplasties and 91 percent of transplantations were performed for optical reasons. Conclusions: pediatric keratoplasty was mainly performed in the study period for visual restoration due to acquired corneal opacities in children aged 6 to 15 years The most common surgical technique was the penetrating keratoplasty(AU)

ABSTRACT

PURPOSE: To compare 2-year clinical outcomes of Descemet's stripping automated endothelial keratoplasty (DSAEK) and penetrating keratoplasty (PK) in patients with bullous keratopathy. METHODS: A retrospective chart review was performed to obtain 2 years of follow-up data of DSAEK or PK at a single center from March 2009 to September 2012. The study comprised 15 eyes of DSAEK and 11 eyes of PK. Outcome measures included best-corrected visual acuity (BCVA), spherical and keratometric changes, central corneal thickness, endothelial cell density, intraocular pressure, and postoperative complications. Graft survival rate was assessed by Kaplan-Meier survival analysis. RESULTS: There were no differences in patient baseline characteristics between the two groups. At postoperative 2 years, better BCVA of 0.69 ± 0.51 logarithm of the minimum angle of resolution (logMAR) was found after DSAEK compared to 0.88 ± 0.48 logMAR after PK. Refractive cylinder in DSAEK and PK was −2.60 ± 1.53 and −6.00 ± 1.05 diopters (D), respectively, and keratometric cylinder was 3.27 ± 3.70 and 6.34 ± 3.51 D, respectively, at postoperative 2 years. The difference of mean spherical equivalents between postoperative 1 month and 2 years was 0.84 D after DSAEK and 2.05 D after PK. A hyperopic shift of 1.17 D was present after 2 years of DSAEK. The mean endothelial cell density at postoperative 2 years was 1,548 ± 456 cells/mm² for DSAEK and 1,052 ± 567 cells/mm² for PK, with a cell loss of 19.96% vs. 52.38%, respectively when compared to postoperative 1 month. No significant difference in central corneal thickness was found between DSAEK and PK (592 ± 75 vs. 563 ± 90 µm, respectively). Finally, the 2-year survival rate did not differ significantly between DSAEK and PK (93.3% vs. 81.8%, respectively, p = 0.344). CONCLUSIONS: Compared to PK, DSAEK provided more stable refractive errors with better visual outcome, lower endothelial cell loss, and a lower rate of graft rejection at postoperative 2 years in patients with bullous keratopathy.

PURPOSE: To report the clinical outcomes of deep anterior lamellar keratoplasty (DALK) when sterile gamma-irradiated acellular corneal tissues (VisionGraft) are used in combination with amniotic membrane transplantation (AMT) for intractable ocular surface diseases. METHODS: The medical records of fifteen patients who had DALK with AMT were retrospectively reviewed. Indications for surgery included ocular burn, bacterial keratitis, herpes simplex virus keratitis, corneal opacity with Stevens-Johnson syndrome, Mooren's ulcer, idiopathic myxoid degeneration of corneal stroma, and recurrent band keratopathy. DALK was performed using partial-thickness acellular corneal tissue and a temporary amniotic membrane patch was added at the end of the operation. RESULTS: All cases that underwent DALK with AMT became epithelialized within 2 postoperative weeks. Twelve patients showed favorable outcomes without graft rejection, corneal opacification, or neovascularization. The other three grafts developed corneal opacification and neovascularization, and required additional penetrating keratoplasty (PK). Unlike the results of previous PKs, there were no graft rejections and the graft clarity was well-maintained in these three cases for at least 8 months after PK. CONCLUSIONS: DALK using sterile acellular corneal tissues in combination with AMT may be a good therapeutic strategy for treating intractable ocular surface diseases because of lowered immune rejection, fibroblast activation, and facilitation of epithelialization. Furthermore, DALK can help stabilize the ocular surface, prolong graft survival, and may allow better outcomes when combined with subsequent PK.

ABSTRACT

Objective: We describe a novel spatula and dissector to facilitate the big-bubble technique in deep anterior lamellar keratoplasty (DALK). Methods: A 29-year-old man who was diagnosed with bilateral keratoconus underwent deep anterior lamellar keratoplasty (DALK). After 350μm partial thickness incision of the recipient cornea, the Bonfadini dissector was inserted at the deepest point in the peripheral incision and could be advanced to the center of the cornea safely because of its "semi-sharp" tip. After achieving the big-bubble (BB) separation of Descemet membrane (DM) from the overlying stroma, the anterior stromal disc was removed. Viscoelastic material was placed on the stromal bed to prevent uncontrolled collapse and perforation of DM during the paracentesis blade incision into the BB. We could detect the safe opening of the BB using the Bonfadini dissector by the leakage of air bubbles into the viscoelastic material. After injecting viscoelastic material into the BB space, we inserted the Bonfadini spatula into the bigbubble safely because of its curved profile and blunt edges. The groove along the length of the Bonfadini spatula enables safe and efficient incision or the residual stromal tissue using the pointed end of a sharp blade while protecting the underlying DM. After removal of posterior stroma, the donor button was sutured with 16 interrupted 10-0 nylon sutures. Results: This technique and the use of the Bonfadini spatula and dissector facilitate exposure of Descemet membrane. Conclusion: The smooth Bonfadini DALK spatula and dissector facilitate safe and efficient completion of DALK surgery. .


Objetivo: Descrevemos o uso de novos instrumentais cirúrgicos para facilitar a técnica de "big-bubble" na ceratoplastia lamelar anterior profunda (DALK). Métodos: Paciente masculino, 29 anos, foi diagnosticado com ceratocone bilateral e submetido à ceratoplastia lamelar anterior profunda (DALK). Após incisão da córnea receptora numa profundidade de 350μm de espessura parcial, o dissector Bonfadini foi inserido no ponto mais profundo da incisão periférica e pode avançar para o centro da córnea com segurança devido à sua ponta semiafiada. Depois de realizar a "big-bubble" (BB) e atingir a separação da Membrana de Descemet (MD) do estroma sobrejacente, o disco corneano de estroma anterior foi removido. Um viscoelástico foi colocado sobre o leito do estroma remanescente para impedir o colapso não-controlado e perfuração da MD durante a incisão na BB com lâmina de paracentese. Verificamos segurança no rompimento do estroma remanescente com o auxílio do dissector Bonfadini, para liberação da bolha de ar da BB através do viscoelástico. Depois de injetar o viscoelástico no espaço da BB, inserimos a espátula Bonfadini neste espaço, o que demonstrou-se seguro devido ao formato curvo e das bordas arredondadas do instrumental. A chanfradura ao longo do comprimento da espátula Bonfadini permite a incisão pela ponta de uma lâmina afiada, protegendo assim a MD subjacente. Após a remoção do estroma posterior, o botão doador foi suturado com 16 pontos interrompidos de fio nylon 10.0. Resultados: Esta técnica e o uso da espátula Bonfadini e dissector facilitam a exposição de membrana de Descemet. Conclusão: A superfície lisa da espátula Bonfadini e dissector, facilitam a realização segura e eficiente da ceratoplastia lamelar anterior profunda (DALK). .

ABSTRACT

Objetivos: describir las características y empleo de la trepanación corneal cónica. Métodos: se realizó un estudio experimental en el Instituto Cubano de Oftalmología Ramón Pando Ferrer de enero a diciembre de 2010. Se realizó la trepanación en diez globos oculares no útiles para trasplante, con un trépano corneal de vacío que tiene como característica fundamental la creación de un corte oblicuo. Se analizaron los parámetros: longitud del corte, grosor corneal, diámetro externo del botón, diámetro interno del botón y ángulo del corte. Se utilizó la estadística descriptiva para expresar los resultados. Resultados: la longitud media del corte fue de 1 776,7 ± 273,7 mm y el grosor corneal medio a nivel del corte fue de 850,0 ± 119,2 mm, con una diferencia entre estos de 926 mm. El diámetro externo medio del botón corneal fue de 7,6 ± 0,2 mm y el interno de 8,5 ± 0,4 mm, con una diferencia de 0,9 mm. El ángulo de corte medio fue de 28,4 ± 2,1°. Se estableció una fórmula para calcular la longitud o profundidad del corte en base a la paquimetría media del círculo en el que se realizó la trepanación para un ángulo de corte determinado. Conclusiones: se obtuvo un corte regular, simétrico y de mayor longitud que el grosor corneal y con el aspecto cónico planificado. Dicho patrón de corte provee una mayor área de contacto y una tendencia autosellante


Objectives: to describe the characteristics and use of the conic corneal trephination.Methods: an experimental study was performed in Ramón Pando Ferrer Cuban Institute of Ophthalmology from January to December 2010. Ten eyeballs not useful for transplantation underwent trephination, with a corneal vacuum trephine particularly designed to create an oblique corneal cut. The cutting length, the corneal thickness, the external diameter of the disc, the internal diameter of the disc and the cutting angle were the analyzed parameters. Descriptive statistics were applied to show the results. Results: the mean cutting length was 1776.7 ± 273.7 mm and the mean corneal thickness at the cutting point was 850.0 ± 119.2 mm , being the difference between them equals to 926 mm. The mean external diameter was 7.6 ± 0.2 mm and the internal diameter was 8.5 ± 0.4 mm, with a difference of 0.9 mm. The mean cutting angle amounted to 28.4 ± 2.1°. A formula was developed to calculate the cutting length or depth based on the mean pachymetry of the circle where the trephination is performed for a determined cutting angle. Conclusions: aregular symmetric cut was obtained, longer than the corneal thickness with the planned conic profile. Such cutting pattern provides a larger contact surface and self-sealing tendency

ABSTRACT

PURPOSE: To compare the clinical outcomes between deep anterior lamellar keratoplasty (DALK) and penetrating keratoplasty (PKP) with same-size grafts in patients with keratoconus. METHODS: Medical records of 16 eyes from 15 patients treated from June 2005 through April 2011 were retrospectively reviewed. Patients with contact lens intolerance or who were poor candidates for contact lens fitting due to advanced cone underwent keratoplasty. The transplantations consisted of 11 DALK and 5 PKP with same-size grafting for keratoconus. Best-corrected visual acuity (BCVA), refractive error, corneal topographic profiling, and clinical course were compared between DALK and PKP groups. RESULTS: The follow-up period was 30 +/- 17 months in the DALK group and 45 +/- 20 months in the PKP group (p = 0.145). At final follow-up, the DALK and PKP groups achieved a BCVA (logarithm of the minimum angle of resolution) of 0.34 and 0.52, respectively (p = 0.980). Postoperative refractive error and mean simulated keratometric index showed myopic astigmatism in both groups without any statistical difference. Corneal irregularity index measured at 5 mm in the DALK group was less than that of the PKP group at 1-year follow-up (p = 0.021); however, at final follow-up, there was no longer a statistically significant difference. Endothelial cell counts were lower in the PKP group than in the DALK group at final follow-up (p = 0.021). CONCLUSIONS: The optical outcomes of DALK with same-size grafts for keratoconus are comparable to those of PKP. Endothelial cell counts are more stable in DALK compared to PKP.
